Community Aged Care

Staying at home longer, and living as independently as possible is the goal we work towards with our clients. Allied Health for the elderly both in-home and in the community is possible because we are mobile. If you are accessing assistance through the my aged care portal or are in need of Allied Health Services that can visit you at home then please contact us.

Home Care Packages

Community Aged Care Packages (CACPs) are individually planned and coordinated packages of care- tailored to help frail older Australians remain living in their own homes. CACPs are flexible and include services to help with individual care needs. We can deliver some of these services direct to you in your home.

Short Term Restorative Care Packages

The STRC Programme provides services to older people for up to 8 weeks to help them delay or avoid long-term care. Read about the STRC Programme, including how it’s managed, approved provider responsibilities, and how to deliver STRC services.

How It Works

We make it easy to get the therapy support services you need, tailored for the ageing population.

We understand it can be overwhelming and confusing to know where to start and how to find services. We work with you from initial contact through the delivery of services to make the process of getting the therapy you need as easy possible.

Here are some of the areas we can help assess you on:

• Activities of daily living and assessment and intervention
• Aids and equipment assessment and recommendation
• Mobility assessment and intervention, including equipment
• Home modifications
• Mealtime assessment, intervention and review
• Urgent swallowing assessment, intervention and review
• Dysphasia assessment, intervention and review

During your initial consultation, we will discuss your goals and availability so we can work on a plan that meets your participant’s needs. We will then set up a service agreement that is based on your NDIS budget and requirements.
